why allopurinol increases acute attack of gouty arthritis

 allopurinol increases acute attack of gouty arthritis  during the early months of allopurinol therapy

because

  • it increases  mobilization of tissue stores of uric acid
  • so thereby causing  hyperuricaemia leading to increased precipitaion of uric acid in joint and acute attack

clinical implication

  • so initial period when giving allopurinol,  always give it along with NSAIDs or COLCHICINE to prevent this attack
